1. Member's Full Name (the insured)
2. Address of the Residence
3. Contact Phone Number
4. 2-3 Photos of the area needing modifications (e.g., bathroom, proposed ramp area, entryways)
5. A Description of Services Needed along with the Contact Information of the Insured's Representative (e.g., care taker, case worker, health plan associate, occupational therapist)

Once we receive your completed form, we will provide a detailed estimate within 24-72 hours. If you require us to come out take pictures this make take up 3 weeks for a preliminary walk through.
